Please print out the Client Health History Form and the Office Policy Form, fill in and bring with you to your appointment. If you prefer to fill the forms out at the office, please show up 15-mins prior to your scheduled time.

NEW COVID-19 APPOINTMENT POLICIES:

  • Health Information and Informed Consent Form will need to be filled out 1-day prior to appointment. Please Fill out form online through THIS LINK and either email back to: leslie@energy-in-motion.net or print and bring with you to your appointment.
  • If you've traveled within two weeks prior to your appointment, especially using public transportation, you may need to wait on scheduling for two weeks in order to make sure you haven't been exposed to COVID-19 in your travels.
  • When you arrive for your appt., please stay in your car or outside and either text me or call me. Once the office has been properly cleaned and set up, I will come and get you.
  • Masks are required for both therapist and client during the treatment. If needed there is PPE available in the office.

OFFICE HEALTH AND SAFETY MEASURES:

  • There is a touch-free thermometer onsite to check temperature upon arrival if needed.
  • All linens will be changed between every client as usual (sheets and face cradle covers) and additionally; blankets and face cradle padding under the face cradle cover.
  • The massage table is now covered with a vinyl layer that will be sanitized after each client.
  • There is now a 240 CADR rated True HEPA Air Purifier in the treatment room.
  • The hand sanitizer and hand soap in the treatment room have been converted to touch-free dispensers.
  • Treatment room chair has been replaced to be cleaned after each client.
  • All surfaces that anyone comes in contact with during appointments will be sanitized between every appointment.

Helpful tips for making appointments...
It can be hard to get in for an appointment at times, but there are ways to find something available. Even though I can be fully booked for 3-4 weeks in advance, I always have cancellations and changes to my schedule. To be put on a waitlist in the scheduling module, look for the day and time frame you're interested in booking, then enter your request where it says, "Don’t see your preferred time? Add your name to our Waitlist" If you choose to add yourself to the waitlist, you will get a notification when an appt. opens up in the time frame chosen. Otherwise, send me a text or email and let me know that you'd like to be put on my cancellation list and I'll let you know when something opens up.

Also, it's easier to cancel than try to get in at the last minute!
So, grab a spot when you see one that works for you in the calendar, even if it's weeks away... Then when you make that appt., put in the notes that you'd like to get in sooner if possible. If you let me know you want to get in sooner, then I will put you on my cancellation list right away. If I get a cancellation, I'll let you know and we can either cancel the appt. you made for later, or keep it!

Cancellation Policy **CURRENTLY SUSPENDED DUE TO COVID-19**
While I understand that cancellations are sometimes necessary, I ask that you please give at least 24 hours notice prior to canceling. This allows the opportunity for someone else to schedule an appointment. Any missed or canceled appointments within 24 hours will be charged the full amount. Should an illness or emergency arise, an exception to this policy will be made at my discretion. Thank you!

Sickness
Massage/bodywork is not appropriate care for infectious or contagious illness. Please cancel your appointment as soon as you are aware of an infectious or contagious condition. If it is within the 24-hour notice period, the cancellation fee will be waived.

No-Shows
Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show,” and should expect to be charged for the “missed” appointment. The only exception would be due to unexpected illness at the time of the appointment.

Late Arrivals
In order to provide consistency and reliability in scheduling for all my clients, I ask that you please be on time for your appointment. Should you arrive late, it may be necessary to reduce your massage to the time remaining, as I now need more time after appointments to sanitize. If I do not have a client immediately following, I may extend your time at my discretion to a full session. If I am able to do that, I will check with you to see if that fits into your schedule before proceeding.
